The Dual-Continuum Model

Traditionally, mental health was understood along a single axis: illness on one end, absence of illness on the other. This framework, while familiar, is incomplete. The dual-continuum model adds a vertical dimension, recognizing that individuals with mental illness can still flourish, while those without a diagnosis may languish in disconnection or low emotional wellness.

Dr.Corey Keyes offers a more complete view of how we understand well-being.

He calls it the Dual Continuum Model of Mental Health.

What Is the Dual Continuum Model?

Instead of placing mental health and mental illness on one line and one continuum. Keyes explains they are two separate things.

  • On the X axis is mental illness. It ranges from serious illness to none.

  • On the Y axis is mental health. It ranges from poor mental health (languishing) to a state called of good mental health (flourishing).

This creates four possibilities:

This model shows us that mental health is more than the absence of illness.

  1. Flourishing with illness – A person has a mental illness and also feels connected, engaged, and well.

  2. Languishing without illness – A person has no diagnosis but feels stuck or empty.

  3. Flourishing without illness – This is the state of strong mental health and no illness.

  4. Languishing with illness – This is the most difficult state, with both low well-being and navigating a mental illness.

    Flourishing: thriving, resilient, and fulfilled—living with vitality and purpose.

    Languishing: feeling flat, unmotivated, or disconnected, even without clinical illness.

This model underscores a profound truth: mental wellness is dynamic, not binary. Fitness professionals, therefore, have a powerful role in fostering environments where flourishing is possible.

The Science Supporting Mental Wellness

Evidence consistently links physical activity, nutrition, mindfulness, and social connection to mental health outcomes:

  • Exercise: Aerobic and strength training reduce symptoms of depression and anxiety. The American Journal of Preventive Medicine (2018) found physical activity lowers depression risk by 25%. The John W. Brick Foundation’s Move Your Mental Health™ Report confirms exercise boosts serotonin and dopamine, enhancing mood regulation.

  • Nutrition: Omega-3 supplementation reduced depressive symptoms (Journal of Clinical Psychiatry, 2017). Gut health interventions, such as probiotics, improved anxiety and depression (Nature Microbiology, 2019).

  • Mindfulness & Meditation: A meta-analysis in JAMA Internal Medicine (2014) showed mindfulness meditation significantly reduced anxiety, depression, and pain. Yoga, too, has been linked to improved mood (Journal of Psychiatric Practice, 2013).

  • Social Connection: Strong networks correlate with lower stress and better well-being (The Lancet Psychiatry, 2020). Connection fosters belonging and resilience, reinforcing both mental and physical health.

Research shows that 78% of individuals now prioritize exercise for mental and emotional well-being over aesthetics, and 94% of personal training clients regularly discuss stress, sleep, and nutrition with their trainers.
